Sacramental Enrolment Form

This form has to be completed to enroll for the After School Program for the preparation to receive the Sacraments of Reconciliation, Eucharist or Confirmation. It is for the candidates who are attending non-Catholic Schools.

Sacrament of Reconciliation / Confession

This form has to be completed and taken to the Church at the Reconciliation Commitment Mass.

Holy Eucharist / Communion

This form has to be completed and taken to the Church at the Commitment Mass of the First Eucharist.

Sacrament of Confirmation

This form has to be completed and taken to the Church at the Confirmation Commitment Mass along with a copy of the Baptism Certificate.

Order of Christian Initiation of Children - OCIC

OCIC Enrolment Form

This form is to be completed and emailed to the Parish Office for those who are under 18 years of age and preparing for all Sacraments together. This may be including Baptism or for becoming a Catholic after Baptising in other Christian churches and to receive further Sacraments (Eucharist and/or Confirmation).

Order of Christian Initiation of Adults - OCIA

OCIA Enrolment Form

This form is to be completed and emailed to the Parish Office by Adults who want to become a Catholic (Catechumen) and to receive Sacraments of Initiation (Baptism, Eucharist and Confirmation) or those who have been Baptised earlier and not received further Sacraments. OCIA program is also for those who wants  to become a Catholic after Baptising in other Christian Churches and to receive further Sacraments (Eucharist and/or Confirmation).
